How often?? When to break and how
How often should one be taking a dose that is a chronic pain patient? Also, I know this has a lot to do with body and tolerance and etc.. Is there a standard dose range that they should stay in? Also, is it possible for someone in pain all day long to taper down When they’re not receiving relief anymore?

We say it a lot but...
As a very general rule, less is more. Meaning that past a certain dose the effects can lessen or even go away. Plus - screws up your tolerance. So the key is to find the dose that works for you.
You always want to start small and work your way up. A gram (or even less) is about all I would recommend ANYONE start with. And if you find yourself approaching three games per dose... probably too much.
Now - the kicker to this is that no batch of kratom is alike, and some batches will be more powerful than others. So the real trick is to find something which really hits for you. This is hard to do at the same time you are trying to calibrate your does. PLUS most of us believe you should not dose with the same batch very often (my own take is to not use it more than once every three days - and if you dose three times a day, which is common for pain relief, that means you are looking for at least six different batches in your rotation).
All that said, I would look here for recommendations for batches that work well for pain relief and do NOT make the mistake of thinking that all names are the same. Not only is "Bali Red" not the same batch from a different vendor, but a vendor's Bali Red #109 might be very different from their Bali Red #110.
Good luck but know that you *can* make it work - it will just take a lot of experimentation (one thing for sure - only take on an empty stomach AND after a bowel movement, and try to time your doses at the same time each day. Not to say it won't work otherwise, but these are things that can increase your chances).
(Oh, and you don't really need to break if you keep your doses reasonable. I've gone for more than a year without a break)>

Kratom is so different for everyone so I can only speak from my own experience and try to give some advice. Like Kelleytoons said above, I'd recommend taking the lowest effective dose that works for you. This will keep your tolerance down. Perhaps start with 1-2g and only work up from there if that low dose isn't working.
As for the number of doses per day, it also varies by person. Personally, I take it 2-3x per day for chronic pain. Usually once when I wake up, once in the afternoon, and then again in the evening. At least 6-8 hours apart. Some strains will work longer than others as well. There are just so many factors at play.
If you take it for pain and it's no longer working, I'd definitely taper off and take a tolerance break to get it working again. I try to take frequent 24 hour mini t-breaks at least once a week. And every couple of months, I try to take a longer break for 48-72 hours.
